Do not and i repeat do not play suicidal hell on earth difficulty after leveling.
It d be an excusable behavior for a game that preloads all textures and ingame settings to be usable upon start when switching options but the game restarts every time you change options.

Locate killing floor 2 from the list right click on it and select properties.
Will all be preloaded.
